Client Services Cancellation Request Form

Please consider this as my notification to cancel my website services. Send me the release form so I can sign it and return it via email attachment or fax to SwissMango. I understand that I will be charged an additional monthly fee at the time of cancellation.
Once the signed form has been recieved by SwissMango the authorization code will be sent to the email address registered on the "Who is" record for the owner to begin the process of domain name transfer to a Registrar of their choice.
